Q. What does Horizons ADHC do?

A. Horizons Adult Day Health Care Center is a Licensed Community Based Adult Services program (CBAS) that provides services to the elderly and adults with illnesses and or disabilities between the hours of 8:00 AM – 5:00 PM.

Q. Does Insurance pay for Horizons Services?

A. For eligible individuals, the following may cover program costs:

  • Medi-Cal
  • Blue Shield Promise
  • Aetna Better Health of California
  • United Healthcare Community Plan
  • Kaiser
  • Molina Healthcare
  • Community Health Group
  • San Diego Regional Center
  • Private Pay

For persons not covered by the above plans, please contact the center’s out reach coordinator worker to discuss payment options.

Q. What kind of transportation arrangements do I need to make?

A. Transportation to and from our center can be provided at no additional cost. Call us at (619) 474-1822 to find out more about transportation availability in your area.

Q. I don’t know if my mother/father will be comfortable at the center with all new people, what should I do?

A. Please contact Horizons to schedule a one time complimentary visit day, family members are encouraged to attend with their loved ones.

Q. My parents do not speak English – will he or she be comfortable at Horizons ADHC?

A. Our staff speaks English, Tagalog, Spanish, Vietnamese, Mandarin, Cambodian, Arabic, Chinese and Russian.
